- Complete the online enrollment form and use your preferred credit card (Discover, Visa, MasterCard, or American Express)
- Mail or Fax print enrollment form to the address/fax number below
- Call Customer Relations at 212.382.6663 to enroll
Please note: You will receive an acknowledgment of your enrollment and an email confirmation once your preferred card has been charged. Your enrollment will remain active until written notification is received by NYCBAR to withdraw from the plan, which you can do at any time. Payments will be charged by the 15th of the month.
